Congenital variant

Displaced ICA runs through middle ear

Enters posterior middle ear via enlarged inferior tympanic canaliculus

Joins horizontal carotid canal via dehiscence in carotid plate

Carotid foramen and vertical segment of petrous ICA are absent

Tubular morphology key observation

(i.e. glomus tympanicum = focal vascular mass)

DO NOT BIOPSY!!!
