Acute optic neuritis. Usually MS or idiopathic optic neuritis.

Examine nerve closely to ensure it is not unusally enlarged. Mild enlargement compatible with inflammation.

Consider glioma, pseudotumor, or other lesion if optic nerve is large. Meningioma with "tram-track".

Consider ischemia in the elderly and radiation if bilateral.
