64 year-old female, incidental finding on CTA.

Carotid Space Mass:

Carotid Body Paraganglioma

Vagal Paraganglioma

Vagal Schwannoma/Neurofibroma

Lymph node hyperplasia.

Paragangloima - derived from primitive neural crest cells in the carotid body; most common paraganglioma (40%)

Presenting symptom -> mass

Catecholamine secretion, malignant tranformation rare

Surgical resection is treatment of choice, low incidence of side effects of < 5cm.

Mass centered in the crotch of the carotid bifurcation

Mass splays the ECA and ICA

Avid enhancement

MR - “Salt and pepper” appearance on pre-gad T1

Angio – Enlarged feeding vessels, prolonged intense blush, AV shunting.
