Typical Imaging Characteristics

T1: hyperintense

T1 + GAD: No enhancement

Low signal intensity on fat-sat images confirms fat

T2 without fat sat: similar appearance to T1 WI

Spinal Lipoma

Subdivided by location

Intradural

Lobulated ovoid/round mass intimately associated with the cord.

+/- Canal widening, local dysraphism

Terminal (Lumbosacral spine)

Attached to distal cord/filum extends through lumbosacral dysraphism.

Thin, “stretched” cord usually tethered

+/- syrinx
